Are you an adult with or a caregiver of someone with an ongoing health condition such as diabetes, arthritis, lung disease, Parkinson’s, heart disease or other chronic condition? If so, consider the Take Charge of Your Health: Live Well, Be Well workshops!

Take Charge of Your Health: Live Well, Be Well and Take Charge of Your Health: Live Well, Be Well with Diabetes are evidence based self-management workshops designed to help you take control of your diabetes or other chronic health conditions. This program is a fun and interactive course that will equip you with the necessary tools to overcome daily challenges to help you maintain an active and fulfilling life.

You’ll get the support you need, find practical ways to deal with pain, fatigue and other symptoms, discover better nutrition and exercise choices, understand new treatment choices, and learn better ways to talk with your doctor and family about your health.

Join a FREE 2 ½ hour Take Charge of Your Health: Live Well, Be Well workshop held each week for six weeks.

Learn from certified volunteer leaders with chronic conditions themselves.

Set your own goals and make a step-by-step plan to improve your health—and your life.

What you’ll learn:

- Eating tips to help you feel better

- Exploring an exercise routine

- Dealing with pain, frustration, isolation, lack of energy

- Managing your medications

- Setting and achieving goals

- Solving problems

- Building a more effective relationship with your doctor

- Communicating with family and friends about your condition

Developed at Stanford University, the research has shown:

- Significantly improved overall health status

- Increased energy levels

- Increased feeling of control of your life

- Improved quality of life

- Reduction of emergency room visits

- Reduction of days in the hospital

- Reduced healthcare costs
